We WELCOME Dealers, Distributors and Resellers.
These are the few simple rules to set you up as a wholesale buyer of our products.

1 • You must be an established retailer, dealer, contractor or builder to qualify.
2 • You must be able to provide proof of your business. Fax  your company letterhead
      with your Federal ID, Business License or Sales and Use Tax  number on it to us at 270-489-2187
3 • You must complete the Application Form below, fill in all areas marked * and SUBMIT it.

Upon acceptance of your business as a resale partner of Jack Wallis Doors, Inc. you
will be sent an email to the address listed in your application file that contains your
USER NAME and PASSWORD to access our confidential price lists.
Please make a note of your user name and password and keep it safe, if you loose your access info we can resend it
to the email address on your application form. If you change email addresses you will need to inform us of the new address.

Our Privacy Policy  At no time will Jack Wallis Doors, Inc. or any of its personnel, affiliates or authorized
dealers provide any of the information you present in your Application Form to any other parties for any
reason without your written consent. We do not divulge any information or give out our mailing lists or email
contacts to anyone. Your privacy rights are a sincere concern of Jack Wallis Doors, Inc.
